Telehealth Consent
Placeholder document for review by counsel before launch. Bracketed items need to be completed.
What telehealth is
Telehealth means a licensed physician reviews your health information and makes decisions about your care using electronic communications, without an in-person visit.
How the review works
You complete a secure online health questionnaire. A licensed physician reviews it and may contact you with follow-up questions. The physician uses their independent medical judgment to decide whether to prescribe.
Benefits and limitations
Telehealth can make care more convenient. It also has limits: the physician cannot perform a physical exam, and decisions rely on the information you provide. If the physician decides telehealth is not appropriate for you, they may decline to prescribe and recommend in-person care.
Your responsibilities
You agree to:
- Provide complete, accurate and truthful information
- Tell the physician about your medical history, medications and allergies
- Follow up with an in-person clinician when advised
- Contact a physician or seek care if you have side effects or concerns
Prescriptions
If a prescription is issued, it is sent to a licensed pharmacy, which prepares and ships your medication.
Privacy
Your health information is protected as required by applicable law, including HIPAA where it applies. See the Privacy Policy.
Emergencies
Telehealth through Dayscripts is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room.
